Purpose:
The day will focus on the critical role you play as a transplant designee to enhance your patients’ lives through transplantation. This multidisciplinary symposium will empower you as healthcare professionals who want to implement the role of a transplant designee. You will specifically learn about renal transplantation as a treatment modality for your CKD patients. This symposium is designed for administrators, nurses, social workers, dietitians, patient care technicians in a variety of healthcare settings as well as other healthcare professionals who work with dialysis patients.

Objectives:
90% of the participants will self-report they plan to incorporate current, evidence-based information regarding transplant into their care and education of patients experiencing chronic kidney or pancreatic disease and actual or potential kidney or pancreas transplant and their families.
Certification:
Upon successful completion of the course, all participants will receive a Certificate of Program completion as Transplant Designees. RNs, patient care technicians, dialysis social workers, dieticians and other attendees will be certified as Transplant Designees according to the guidelines established by Quality Insights Renal Network Three. Certification will be valid for 12 months. Other healthcare professionals are encouraged to attend. certificate, participants must stay for the entire program. Evaluations must be completed on-line. Certificates will be
sent automatically when evaluation is complete.
The evaluation must be completed within 14 days of the program to receive a contact hour certificate.

This program is approved for 5 general social work credits by Hackensack Meridian School of Medicine. This is in accordance with NJ State Board of Social Work Examiners NJAC 13:44G: 6.4 #7 To obtain your certificate you must click the SW CEU link in the zoom chat during the presentation to validate your attendance. The evaluation link will only be available during the presentation. Your certificate will be automatically emailed to you upon submission. Full attendance is mandatory and will be verified.
